About Community Partners:
  • Successfully launched in the 1990's in five states and has been in Indiana since 2000.
  • Community Partners is now available in all 92 counties.
  • Free and voluntary.
  • In-home visitation program.
  • Community liaisons (case managers) assist families in developing specific family
    centered goals to reduce stress and help find the balance in life.
  • Liaisons to help families discover and connect with local community supports and services.
Why Community Partners?
  • Everyday life is stressful; trying to find the balance in life is hard.
  • Being a parent is a tough job!
  • Sometimes children need help getting connected with youth programs and resources.
  • Families need options and choices before there is a crisis.
  • All families have value and strengths.
  • Because families are best supported when the community works together.
Who is eligible?
  • Families who are not actively involved with the Department of Child Services
    or Healthy Families.
