- Successfully launched in the 1990's in five states and has been in Indiana since 2000.
- Community Partners is now available in all 92 counties.
- Free and voluntary.
- In-home visitation program.
- Community liaisons (case managers) assist families in developing specific family
centered goals to reduce stress and help find the balance in life. - Liaisons to help families discover and connect with local community supports and services.
- Everyday life is stressful; trying to find the balance in life is hard.
- Being a parent is a tough job!
- Sometimes children need help getting connected with youth programs and resources.
- Families need options and choices before there is a crisis.
- All families have value and strengths.
- Because families are best supported when the community works together.
- Families who are not actively involved with the Department of Child Services
or Healthy Families.
- Families are in charge of the process.
- Focus on your strengths.
- Support/assist you in accomplishing your goals.
- Connect you with resources and programs that you may not realize exist.
- Stay with you through difficult times and help you get back on your feet.
