Filling In Information using Adobe Acrobat Reader:
   
  1. Open the Fill-in Form document by clicking on the form's link. (Note that you must have the free Adobe Acrobat program installed on your computer. You may download and install this program by clicking on the Acrobat icon on our CCDF webpage).
 


   
 

 

2. Using your mouse, click into a blank space on the form. A cursor will
     appear.
 Type in the desired content.

    You can advance to the next space in the form by pressing the tab
    key or clicking into the next space on the form with your mouse.

 


   
 

  3. Clicking inside a checkbox will cause a check (or "X") to appear. Clicking
     again will remove it.
 


   
   
 

4. Print and sign the document if it has a signature block(s).

    Saving the file will only save the form. The filled-in information WILL NOT
    be saved.
